Please use this form to submit adjustments or cancellations to ongoing appointments you have scheduled with All Abilities.

    Please note that our cancellation policy applies and 24 hours notice is required to cancel ongoing appointments in order to avoid a cancellation fee being charged.
  • Do you wish to cancel all ongoing appointments with All Abilities Therapy?
  • Do you wish to suspend your ongoing appointments with All Abilities Therapy? Please note that your appointment time cannot be held for you. All Abilities will await contact from you to discuss resuming services at your earliest convenience.
  • For clients accessing NDIS funding - following submission of this form and cancellation and/or suspension of appointments with All Abilities Therapy, we will cancel your NDIS Service Booking and release the funds set aside to cover the intended sessions. Please be advised this may take up to 30 days and you will be unable to allocate this funding to another agency until the cancellation has been completed. This is due to the way the NDIS portal is organised and not a function of our clinic or its policies.
